About The Position We are seeking a skilled Geriatric Nurse Practitioner for a 78-day locum tenens assignment in Colorado, starting 06/23/2025. This part-time position offers flexibility, with the provider setting their own schedule for 10-20 hours per week, typically over 2-3 days. The assignment involves providing primary care services to residents in up to three assisted living facilities in the Denver area. Responsibilities include managing various primary care issues for geriatric patients, with the ability to refer complex cases to local hospitals or residents' primary care physicians. This opportunity has strong potential for extension and may evolve into a full-time position. Proficiency with Chart Path EMR is required. Job Details: Location: Colorado Facility Type: Assisted Living Facilities Start Date: 06/23/2025 LOA: 78 days Provider Type Needed: Geriatric Nurse Practitioner Schedule: Flexible, 10-20 hours per week (2-3 days) Responsibilities and Duties: • Provide primary care services to geriatric patients in assisted living facilities • Conduct regular health assessments and physical examinations • Diagnose and treat common illnesses and chronic conditions in elderly patients • Prescribe medications and develop treatment plans • Coordinate care with other healthcare providers and specialists as needed • Educate patients and their families on health maintenance and disease prevention • Maintain accurate and detailed medical records using Chart Path EMR • Manage and prioritize patient care across multiple facilities • Adapt to the unique needs of each assisted living facility • Participate in care planning meetings and collaborate with facility staff Additional Information: • Must be licensed to practice in Colorado • Experience with geriatric patients is required • Familiarity with Chart Path EMR is necessary • Opportunity for assignment extension and potential full-time position • Ability to work independently and manage time effectively across multiple locations Apply now to learn more!
